We are excited to share this opportunity with all of you to share your health needs (anonymously) to help shape healthcare policy and inform the State and others about what are the current needs of the people in Maine. The Maine Shared Community Health Needs Assessment (CHNA) wants to hear from people living in Maine about their health and well-being both individually and for the communities where they live. This is one part of our community engagement effort, which also includes focus groups and key informant interviews.
Goal
This survey will be used to validate key themes from our other community engagement initiatives and to gather additional information from a wider array of people in Maine about their health and well-being and that of their community.
Vision
The Maine Shared Community Health Needs Assessment helps to turn data into action so that Maine will become the healthiest state in the US.
Mission
The Maine SCHNA is a dynamic public-private partnership that:
- Creates Shared Community Health Needs Assessment Reports,
- Engages and activates communities, and
- Supports data-driven health improvements for Maine people.

To thank you for taking the time to do this survey, you will have the option to enter a raffle. Ten (10) randomly selected people will each win a $100 gift card. The contact information you share to enter the raffle will be kept separate from your survey answers.
